This is a simple web application I made. The goal is to fix the problem of having to carry around tens of loyalty, discount, and club cards every time I go shopping. I finally broke down and created this page when I realized my wallet was way too thick and 3/4 of the thickness was from the various supermarket and other loyalty cards.
Follow the instructions on the main page to create your own card. Print out the generated card, cut it out and fold it together to make your one new card to rule them all.
Although those Blockbuster cards do have a barcode on them, that barcode is also your account number. Best to not be shooting those around the internet...
The same goes for Hollywood Video too
The following cards have barcode issues so I can't add them right now. If you own a barcode scanner or otherwise feel ambitious, I've tried to include the barcode type when available below so you can try your luck on the advanced page for some of these. (Note for code39, you need to put a * at the beginning and end of the number when using the advanced page.)
Sometimes this is a problem, and the simple solution is to cover all the barcodes but the store you're at with your fingers.
The better solution is to print this Just One Club Card windowed-sleeve cutout (PDF), cut it out, and construct a sliding, windowed sleeve for your new just one club card. If someone could turn this sleeve part into a quick instructable for me, I'd be very appreciative, just email me.
Your sleeve should look something like this. (but hopefully better since I'm a bad cutter.)

I keep the data only long enough to make sure the site is functioning properly. e.g., are any club card numbers in a different format than I'm allowing?, Is anyone using DSW, can I get rid of it? Is any store causing the site to crash, etc.
I periodically purge the log file that contains the numbers, and I solemnly promise never to share the numbers with anyone.
Utimately it is your responsibility to only use club cards that aren't sensitive. However I'll also do my best to purge the logs periodically, and keep the logs secure before they're purged. And to the best of my knowledge I try not to offer any club cards that use account numbers or other sensitive information.

However the advanced page does just this. For each card you want to add, enter the number, and then try each barcode type (barcode types are at the very bottom of the drop down lists) until the image matches that barcode you see on the card. You can fill in the store name in the blank area.
A few users have reported this issue. Here are a few things to try:
Try printing the card on a different printer. Some printers especially HP's won't print all the barcodes.
Try printing the card to PDF first and then printing the PDF file from your printer. Here's one program that will do this on Windows.
